With the Malfunction Skull, every time you respawn a random part of your HUD will be missing. You find the Skull on the Control Room mission. To get it, go into the underground room that is through the tunnels (the big room). There are pipes in the center that stick out both sides. You can find the Skull on the side of the gorge you are on, from where you first enter.
