You need the Undead Nightmare DLC for this ***and it contains spoilers***
When you approach Seth about the glass eye, he says to John "Either enjoy em or become on of em".
Then after finishing it you see John's grave and a short clip plays showing Seth steal the Aztec mask you needed to stop the zombie apocalypse.
